Every scar should be considered unique and needs a customized approach. You do not have to put up with scars from acne, burns, bad surgery outcomes, or an injury anymore. The fresher and younger the scar, the easier it is to improve it. The hardest scars to treat are old and thickened but that does not mean they cannot be improved. Patience is the key to seeing the results you want. Here are some of the tools that Dr. D uses for scars:
The most useful of all products to smooth out scars is Tretinoin, commonly known as Retin A. This is a vitamin A derivative that stimulates and turns over the skin to help reshape and smooth scars. Daily applications of tretinoin for 4 to 6 weeks especially right after the scar occurs can take care of it without any other interventions. Tretinoin will make the area red, peeling and irritated which indicates that the medication is working. Other topical treatments include silicone sheeting and vitamin C creams.Â
Injections of certain medications like steroids and 5FU can soften and smooth out scars. This approach is usually the first line for older scars that are raised, hard, or thickened. The injections are well tolerated and usually show results after the first treatment session. It can take several injection sessions to see the final results.
This is an oldie but goodie treatment for depressed acne scars. These scars are called “saucers” due to their appearance and will smooth and flatten out when you stretch them. Subcision is a technique of breaking up the scar tissue that is pulling the scar down to allow it to flatten and smooth out. This is a simple procedure using local anesthesia and a special tool that only takes about 15 minutes. Usually, 3 to 4 sessions will be needed to see the final results of subcision for acne saucer scars.Â
These lasers treat scars by decreasing redness, decreasing scar volume, and improving scar texture. Vascular lasers work best when used early in the treatment of scars. Scars that are red or purple will respond to excel® V or Laser Genesis. The treatment is fast and painless and usually needs to be repeated 2 to 4 times to see the best results.
This is an ablative laser treatment which vaporizes and removes small amounts of tissue. This creates improved scar texture, firmness, and smoothness. It is useful for acne scars that are so-called “boxcars” as well as for burn scars. Ablative fractional lasers are better in treating scars than non-ablative lasers.Â
Chemical peels can smooth scars, especially large pores and acne scars. The composition and number of peels required are dependent on the severity of the problem. Â
Microneedling is a relatively new treatment option that is less invasive than some other treatment options. It is just starting to be studied so the jury is still out as to its effectiveness. A series of 3 to 5 sessions combined with platelet-rich plasma (PRP) can result in a 50-70% in some scars.
Bellafill® long term filler is FDA approved to smooth out saucer acne scars by filling them in to lift the depressed part of the scar. Bellafill® is composed of collagen and PMMA microscopic beads to continually stimulate collagen. This gives it a long term improvement by several years. Other hyaluronic acid fillers can be used as well however do not have the long-term benefit that Bellafill® does.Â
